We make every attempt to have accurate insurance information prior to your scheduled appointment. Make sure to have your insurance ID card on hand when scheduling your appointment. Some procedures require a consult prior to surgery to ensure coverage. If your insurance does not cover your procedure, you are responsible for the treatment cost.
